Upper limb regional anaesthesia and altered coagulation function.

چکیده
Editor—We thank Dr Dolenska for her letter and completely agree that the finding with respect to the predictive value of patient age is highly counterintuitive. Age is generally accepted as a powerful predictor of survival in many contexts. However, our results would suggest that it is the decline in fitness with advancing age which is the real predictor, rather than age per se. We do not believe that the finding could be erroneous. Specifically, when the trillions of models potentially explaining our data were distilled down to the few hundred very best models via the Bayesian model averaging process, 66% of those models predicted survival without requiring the input of patient age. The general linkage between age and fitness makes age a reasonable surrogate for fitness in the absence of fitness data. However, where one has access to such data, it renders age largely irrelevant. The day may be fast approaching where life insurers require a cardiopulmonary exercise testing before quoting a premium.

Regional anaesthesia for upper limb trauma: a review
Trauma to the upper limb is extremely common. Many of these patients present for surgery and anaesthesia as an emergency case, often complicated by other injuries and requiring effective perioperative analgesia. Although the use of regional anaesthesia for elective upper limb surgery has greatly increased, there is a lack of readily available information on the use and potential benefits of reg...
متن کاملCervical epidural anaesthesia for thyroid surgery.
BACKGROUND Cervical epidural anaesthesia is a regional anaesthesia technique which has been used for upper limb surgery, upper thoracic wall surgery, carotid artery surgery and neck dissections. Anaesthesia for thyroid surgery can be complicated due to the altered functional status of the thyroid or its large size. OBJECTIVE This prospective study was designed to assess the effectiveness and ...
متن کاملUltrasound-guided regional anesthesia for upper limb surgery.
PURPOSE The purpose of this module is to review the main ultrasound-guided approaches used for regional anesthesia of the upper limb. PRINCIPAL FINDINGS The anatomical configuration of the upper limb, with nerves often bundled around an artery, makes regional anesthesia of the arm both accessible and reliable. In-depth knowledge of upper limb anatomy is required to match the blocked territory...
متن کاملRegional anaesthesia in a Duchenne muscular dystrophy patient for upper extremity amputation.
Duchenne muscular dystrophy (DMD) is the most common of the neuromuscular disorders. DMD is usually a challenge for the anaesthesiologist, with poor cardiac function, a high risk of developing rhabdomyolysis, and the probable life-threatening complications of general anaesthesia. To avoid possible morbidity associated with general anaesthesia, we applied a supraclavicular blockade. In this repo...
متن کاملClinical evaluation of upper limb function: Patient’s impairment, disability and health-related quality of life
Musculoskeletal disorders substantially impacts physical activity, mental state, and quality of life (QOL). Generally, comprehensive assessment of upper limb function requires measures of impairment or disability as well as health-related quality of life. A growing number of outcome instrument have been introduced to evaluate upper limb function and disability, and these measures can be categor...
